Centuries ago we would not have been alive past the age of 50 so it would have been fine that our organs stopped producing hormones. Now, around the menopause, we experience a drop in our hormones; oestrogen, progesterone, testosterone & DHEA. This contributes to skin ageing, resulting in the skin becoming thinner, an increase in the depth & number of wrinkles , increased dryness & decreased plumpness / elasticity.
In the aesthetic industry this has implications & gives us another tool to improve the skin. The skin is sensitive to the actions of oestrogen, progesterone & testosterone. Oestrogen & progesterone stimulate collagen production & also inhibit MMP-1 which causes collagen & elastin to be clumped & disorganised. Testosterone causes a reduction in collagen production. Oestrogen deficiency also means the skin is less protected causing impaired wound healing, hair loss, pigment changes & skin cancer.
With this knowledge, as well as all the classic symptoms of the menopause such as hot flushes, loss of libido, loss of confidence & low mood, it is crazy that more women do not take hormone replacement. The reason is largely due to misinformation about hormone replacement. Doctors & women are very aware of the studies that showed that there is a significant increase risk of breast cancer in women taking hormone replacement therapy (HRT) for more that 5 years. What, however, is not widely known is that if a woman takes Bioidentical & not synthetic progesterone this risk is not there. Women should be asking for Utrogestan as their progesterone & also having testosterone levels checked as a small amount of testosterone can not only improve libido & confidence, it can improve the skin. Offering Bioidentical hormone replacement therapy to aesthetic patients is the way to truly provide holistic care.
